HEALTH CHECKS

Prevent today for a healthier tomorrow.

The purpose of this health assessment is to help identify any risk factors that may require further health management. In addition to assessing a person’s health status, a health assessment is used to identify a broad range of factors that influence a person’s physical, psychological and social functioning.

Our friendly and professional nurse will undertake a comprehensive and confidential health check.

If you are aged 75 years and older and live at home, you are eligible for a bulk-billed annual health assessment.

The annual health assessment involves:

• Information collection, including taking a patient history and undertaking or arranging examinations and investigations as required;

• Making an overall assessment of the patient;

• Recommending appropriate interventions;

• Providing advice and information to the patient;

• Keeping a record of the health assessment, and offering the patient a written report about the health assessment, with recommendations about matters covered by the health assessment; and

• Offering the patient’s carer (if any, and if the GP considers it appropriate and the patient agrees) a copy of the report or extracts of the report relevant to the carer.

Components of the health assessment

•  Specific components of the health check includes an assessment of the patient’s:

Continence;

• Immunisation status for influenza, tetanus and pneumococcus;

• Physical function, including the patient’s activities of daily living, and whether or not the patient has had a fall in the last 3 months;

• Psychological function, including the patient’s cognition and mood;

• Social function, including the availability and adequacy of paid and unpaid help, and whether the patient is responsible for caring for another person;

• Measurement of the patient’s blood pressure, pulse rate and rhythm;

• Any need the patient may have for community services;

• Social wellbeing of the patient;

• The patient’s oral health;

• The patient’s nutrition status.

Above all, our nurse will work closely with you and your General Practitioner. Therefore, our nurse will co-ordinate any necessary follow-up consultations or supports to help you manage your health in the future. This health assessment is provided at no cost.

If you are aged between 45-49 year old, you can book a free health check with us.

Entering middle age presents a whole new host of health challenges for many people. The Australian Department of Health categorises 45-49 year olds as the demographic most at risk of developing a chronic illness.

Why do you need to get a 45-49 year old health check?

Between the ages of 45 and 49 years old is when, statistically, you have a good chance of showing signs of the development of chronic disease. Unfortunately chronic diseases can have a significant impact on your health and well-being later in life. Your GP may make recommendations to help you avoid health problems later in life.

If you haven’t been particularly conscious of your health in the past, then this health check is critical. For people who display risk factors for developing chronic disease, a 45-49 year old health check is even more important.

What’s involved in a 45-49 year old health check?

Our health assessments are a comprehensive and full health check of your general overall physical and mental health.

A mental health assessment and physical examination and health assessment is vital to identifying and preventing the onset of any chronic illness. These checks include a thorough examination of your diet, exercise, family history, and the review of any medication you may be taking. It may also include immunisations, type 2 diabetes risk evaluation tests and/or screening tests considered necessary for your age and/or health history.

Other components of the health assessment may include:

• Undertaking interventions and referrals where clinically needed;

• Delivering advice and information, including strategies to help improve a patient’s lifestyle and behavioural patterns;

• Medication review and management if needed;

• Write and record the health assessment, and provide the patient with a written copy of the health assessment underlying the recommendations covered in the appointment.

Health assessments run for approximately one hour, including 30 minutes with a practice nurse followed by 30 minutes with your GP.
